**Job Summary**
We're seeking an Experienced Licensed Social Worker to join our team on a part-time basis. This remote role requires the ability to work from anywhere in the U.S. and will involve working Monday-Friday, 8am-4:30pm Pacific Standard Time. As a Licensed Social Worker, you'll be responsible for researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work to empower members to make positive lifestyle and medical choices and collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
**Responsibilities**
* Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Empower members to make positive lifestyle and medical choices
* Collaborate with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Conduct telephonic or electronic exchanges with members to assess their needs and provide support
* Develop and implement plans to address members' needs and improve their health outcomes
* Maintain accurate and up-to-date records of member interactions and progress
**Qualifications**
* Must have an active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
* 2+ years of experience in medical social work or case management
* Masters degree in social work or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
* Ability to maintain confidentiality and handle sensitive information
