MDS Coordinator, Hybrid - MMG

We are adding another MDS coordinator to our team, work remotely/onsite with our MDS team! TITLE: MDS Coordinator $40-$45 wage range Job Summary: This position is responsible for assuring the timely completion of accurate assessments, interviews, and interdisciplinary care plans that meet Federal and State guidelines. Included is identifying resident acuity and needs, helping to determine specific care needs and communicating needs to responsible parties. Qualifications: Must have a current/active RN license with the State of Minnesota and have 3 or more years of MDS experience. Must keep up with CEU’s for license renewal. Must have the ability to communicate and comprehend in order to gather information from residents and their families. Must be able to educate and train employees with diverse ages and backgrounds on the correct way to code resident’s ADLs. The MDS Coordinator is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.
