[Remote] Patient Enrollment Specialist

The Patient Enrollment Specialist serves as a trusted advocate for patients, guiding them through the enrollment process for home-based care services while ensuring they are fully informed and supported. Responsibilities Initiate outreach to patients and caregivers to explain the benefits of home-based care services—including clinical, emotional, and logistical advantages Build rapport with hesitant or uncertain patients; address objections, answer questions, and provide personalized support to encourage enrollment Use motivational interviewing techniques and empathy-driven communication to support patients through their decision-making process Review and respond to incoming referrals; conduct proactive outreach to eligible patients in a timely manner Guide patients through the enrollment process from start to finish—including eligibility verification, paperwork completion, and consent collection Coordinate closely with care teams, discharge planners, physicians, and social workers to ensure timely communication and continuity of care Reconnect with undecided patients to reassess readiness and continue relationship-building until a decision is reached Maintain accurate records of outreach efforts, patient concerns, and enrollment status in EMR systems Skills High school diploma or equivalent required 1–3 years of experience in healthcare outreach, patient education, admissions, or other patient-focused position Strong verbal and written communication skills with a persuasive, empathetic approach Ability to clearly communicate medical and logistical information in patient-friendly language Confidence in handling objections and navigating sensitive conversations Passion for helping vulnerable or underserved populations access care Associate or bachelor's degree in healthcare, communications, social work, or a related field Prior work in home health, hospice, managed care, or chronic condition programs Comfortable working with EMR systems, Athena Familiarity with motivational interviewing or patient-centered communication techniques Bilingual abilities (especially Spanish) Benefits 401k matching Comprehensive medical, dental, and vision coverage No-Meeting Fridays Monthly company holidays Access to mental health resources Generous flexible time-off policy Employee Stock Purchase Plan (ESPP) offering discounted equity opportunities Reimbursement for office setup expenses Monthly cell phone & internet stipend Paid parental leave for all new parents Company Overview Clover is a healthcare technology company helping members live their healthiest lives with its Medicare Advantage plans.
