Overview: The Remote Patient Monitoring (RPM) RN plays a critical role in supporting the Care Management Program by collecting and analyzing biometric data to monitor patient health status. This role involves telephonic clinical assessments, patient education, and proactive care coordination to ensure timely interventions and optimal health outcomes. The RPM RN collaborates with internal teams and external partners to drive program success and continuous improvement. Qualifications: Licenses and Certifications: • Active and unrestricted Registered Nurse (RN) license in New York State. Education: • Associate's Degree in nursing required • Bachelor of Science in Nursing (BSN) preferred; graduation from an accredited nursing program required. Work Experience: • Minimum of 3 years of experience as a Registered Nurse (RN). • At least 2 years of experience in care management; telehealth experience is a plus. • Clinical expertise in geriatric care preferred. Skills: • Strong analytical and clinical assessment skills. • Excellent communication and patient engagement abilities. • Proficiency in telehealth platforms and remote monitoring tools. • Bilingual skills may be required based on operational needs.
