RN/LPN MDS Coordinator/Restorative Nurse, Remote Hybrid

This is a hybrid remote position.MDS Coordinator/Restorative Nurse Requirements:• Applicant must possess strong communication, clinical, and organizational skills.• MDS experience preferred.• Current/active RN/LPN licensure required.MDS Coordinator/Restorative Nurse Job Duties:• Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.• Assist with performing all admitting and discharge assessments, and complete all other necessary paperwork.• Help monitor all aspects of patient care.• Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff.• Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained.• Coordinate and prepare MDS assessments for all residents.• Oversee the transmission of MDS resident assessments.• Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.• Participate in Care Plan meetings with residents and family members.• Be aware of residents’ exit to and reentry from hospitals.• Oversee restorative nursing program.MDS Coordinator/Restorative Nurse Benefits*:Dover Nursing and Rehabilitation offers competitive pay and a comprehensive benefits package.
